Carers' Support East Kent is a charity that provides Carers with the information and support they need. Our services are available to people looking after a relative or friend, who due to physical or mental illness, age related difficulties, disability, or an addiction, cannot manage without their support.
Caring can be rewarding but also challenging. Each situation is different, but we know that caring brings practical, physical, and emotional challenges. We care about the health and well-being of Carers and provide information, support, and signposting/referral to services to help with those challenges.
About the role
A fantastic opportunity has arisen for a Team Leader - Community to join our team. Reporting to the Carers Service Manager and working as part of the Community Team. We are looking for a self-motivated, flexible, and experienced Team Leader to provide first line management to a team of Carer Support Coordinators and also provides direct support to Carers to help them navigate through health and social care systems and access community resources. The post holder is responsible for providing appropriate up to date information, and support to both staff and Carers. This post is vital to the delivery of our service contracts and makes an important contribution to the range of Carers' services provided.

Key Responsibilities include:
Provide first point of contact to a team of Carers Support Coordinators, providing information, and support about queries or problems in relation to individual cases
Support the Carers Service Manager in the implementation of change, revised practice, processes and reporting systems to facilitate continual improvement
Support and manage team members to deliver organisational goals and objectives
Implement performance management policy, review, and develop as appropriate
Allocate Carers Assessments to team members and quality assure Carers
Assessments once completed for temporary cover of the Team Coordinators
Analysis staff performance data on a regular basis with the Carers Service Manager
Provide regular monthly supervision with your team members
Authorise Carers one off payments and provide first line approval for applications for a Direct Payment
Working on the KCC database Mosaic to monitor reviews and support plans to ensure actions are in place to remove historic reviews/ support plans using POWER BI
Work with the Carers Service Manager and other Team Coordinators to plan activities
Allocate outreach visits and other undertakings to team members
Provide examples of impact of support to Carers using case studies and evaluation tools
Contribute to Carers newsletters and promotional materials
Carry out risk assessments as required
Authorisation of Service Delivery orders and Primary Care rapid Access referrals temporarily in the absence of the Health Team Leader.
Contribute weekly to the safeguarding reviews meetings with the Safeguarding Manager.
Manage incoming feedback, positive or negative and complaints from staff or Carers.
Attend Carers Assessments and one - one session with team members on difficult cases.
To be successful you will have:
Excellent communication, interpersonal and written skills.
Experience of the health and social care sector essential for this role.
