This role will involve helping individuals as they move through key changes in their lives, home to independent living, or supporting people with disabilities to adjust to new routines.Key Responsibilities:

  • Provide person-centred support to individuals navigating life transitions.
  • Assist with developing and implementing tailored support plans to address unique needs.
  • Offer practical help and emotional support to enhance independence and well-being.
  • Build strong, trusting relationships with individuals, their families, and other professionals.
  • Assist with daily living tasks, including budgeting, meal preparation, and personal care as needed.
  • Promote inclusion, advocacy, and participation in the community.
  • Support individuals to develop skills that increase their confidence and self-sufficiency.
  • Encourage individuals to express their preferences, values, and needs.
