The Role
An exciting opportunity has arisen for a motivated and skilled Social Worker/Care Coordinator to join the top performing Early Intervention in Psychosis Team in Bolton.

The Early Intervention in Psychosis team offers the provision of assessment and intervention for people 14 years to 65 years who may be experiencing their first episode of psychosis. Care co-ordination within the Care Programme Approach is offered with multi-disciplinary provision. Interventions include medical reviews, psychology assessments/interventions/1:1 and group interventions/family therapy/Access to employment support/Physical health interventions and Support Time Recovery Workers.
The function is to offer support within the community at the first episode of psychosis to promote mental and physical health and well-being and recovery.
The main duties of the role are to manage a defined caseload of clients within the Early Intervention in Psychosis Team (EIT) with complex physical and mental health needs in the community. You will use evidence based/client centred principles to assess, plan, implement and evaluate interventions and record all interventions, in line with CPA process and the EIT operational procedure.
The role will require you to provide professional advice and/or supervision for junior staff, support staff, volunteers and students, who you will also regularly supervise during their field placements. To participate in the planning, development and evaluation of the EIT service and holding responsibility for defined projects.
You will be part of the duty assessment rota operated by the EIT Team. You will also be part of the extended hours service, which would include weekend and bank holiday working on a rota basis.

About You
You should be able to work flexibly, be creative, be committed and able to adapt positively to change. You will be able to effectively work using a holistic approach towards assessment and treatment, working in partnership with service users and their carers to promote independence and recovery. You will need to be able to work effectively as a member of a multi-disciplinary team, respecting the input of all the professions and professionals within this.
You will be joining a client centred, diverse and friendly team, who prides themselves on delivering high quality, evidence-based interventions for those experiencing first episode psychosis.
