About the Role:
This is an exciting opportunity for experienced Social Workers to support the functions of the hospital discharge team. This is a challenging position, and you will undertake a range of assessments and interventions to ensure those adults with care and support needs and their families and carers' achieve their desired outcomes.

The role is also supported by access to CPD opportunities which are tailored to individual and service needs; these include Best Interest Assessor and Practice Educator training modules.
About You:
You must be a qualified Social Worker who is registered with Social Work England. Have excellent communication skills and possess the drive, ambition, and commitment to practice within a busy, pro-active, and supportive team, who actively maximise the independence and wellbeing of the people we support.
You are able to demonstrate an applied knowledge of Adult Social Care legislation, hospital discharge statutory guidance and best practice, and you are willing to undertake further learning and development to support your on-going progression.
