Role Overview:

An opportunity has become available in Oldham\xe2\x80\x99s Older Adults integrated mental health service for a passionate and enthusiastic social worker. The vision of the team is to help the people of Oldham live their best lives possible through promoting independence; by providing high quality strength based, outcome focused support for residents that is appropriate for their needs. The CMHTs operate an assessment and Care Coordination function working with those over 65 who have complex mental health needs.

Our integrated teams consist of Social Workers, Community Mental Health Nurses, Community Support Workers, AMHPs, Occupational Therapist and Community Psychiatrists You will be joining a service area where strong collaborative relationships already exist between partners such as Adult Social Care, Care Home Liaison , Intensive Home Treatment Service, Inpatient Wards and ICB colleagues.

Oldham Mental Health services is embarking on significant transformation work, with a focus on developing CMHTs and a new mental health offer aligned to our Primary Care Networks, with a strong focus on place based support for people where it is needed.

Key Responsibilities:

As a Social Worker in the CMHT your main duties will be to provide an assessment, support planning and review service, underpinned by professional knowledge and legislation in order to ensure that:

  • Significantly complex individuals with Dementia or a functional mental health need, are supported appropriately including, contributing to Mental Health Act assessments when needed, undertaking mental capacity assessments and best interest processes
  • Vulnerable adults are safeguarded and are supported to live well with risk;
  • Strength based conversations that result in person centered outcomes and support requirements are identified.
  • Coproduce support plans that are strength based and outcome focused
  • People are encouraged and enabled to live healthy and independent lives.
  • Care coordinate a case load that reflects your experience, knowledge and skills.
  • You should have a good knowledge of the MHA, MCA and safeguarding and be able to utilise relevant legislation.
  • You will be leading on and contributing to risk assessments and risk management plans.
  • When required support all team functions, assessment, Care Coordination and the duty system
  • Contribute Social Work expertise and challenge ideas and decisions based on a person\xe2\x80\x99s age.
If an AMHP is appointed, there is a requirement to support the AMHP rota.

In order to apply for this exciting position, we need you to have:
  • A DIPSW or degree level social work.
  • Social Work England registration
  • Willingness to improve practice of self and others
If applying at Grade 7 post, you will have an appropriate post qualifying award such as consolidation module, practice educator stage 1 and/or 2, Best interest Assessor or equivalent with significant experience.

If an AMHP and contributing to the AMHP rota as required salary will be at a grade 8.
