The Transition Service supports young people aged 17.5 up to 25 with severe and enduring learning disabilities (who may also have physical disabilities) and their families through the process of transitioning from childhood to adulthood in a planned and personalised way.
Moving from childhood to adulthood can be a difficult time for any young person. For young people with a disability, there are extra demands and difficulties to be faced. The Transition Service uses person-centred approaches as the team focuses on developing plans for young people as they move into adulthood using the Preparing for Adulthood model. We treat every young person and their family with respect, acknowledge their right to make choices about support that they need and enable young people to be as in control as they can be of the changes that they need to make.
We have a great opportunity for a motivated and proactive registered Social Worker who is committed to ensuring that the service supports young people and their families to make choices about their future in an individual and timely manner.
To fully support young people as they transition into ASC we work closely with Children's Disability Social Care, Education and Health Services. Knowledge, experience, and an understanding in this area of social work would be advantageous as will experience of working with young people with disabilities and their families. If you don't already have this breadth of experience, we can train you. The Transition service is part of a large Council which actively promotes social workers CPD by offering regular training, appraisals, and supervision.
Our hybrid working arrangements allow for a mix of being home and office based throughout the week. This post covers clients living across the county.
We are looking for candidates who:
Are keen to develop further as a social worker.
Can work as part of a team but also work on their own initiative.
Have effective communication skills and feeling confident in discussions with other professional disciplines.
Have an ability to meet travel requirements of the post.
Have a commitment to training and personal development.

Working Pattern: Monday-Thursday 8:30am-5:00pm and Friday 08:30am-4:30pm.
