Active Luton is looking for a Link Worker to join the Family Hubs-Maternal Social Prescription team working closely with GPs and other healthcare staff.
Active Luton has an exciting opportunity available for a passionate, enthusiastic, dynamic and motivated individual to support pregnant women, new parents and their newborn during the first 1001 days of life, providing 12 to 18 weeks of wellbeing and social prescribing interventions through our Family Hubs network.
Social prescribing empowers people to take control of their health and wellbeing through referral to Link Workers. The programme aims to work proactively with the community to support individuals to access and connect to appropriate services and activities to empower people to take control of their health and wellbeing.
The successful applicant should have experience of working in health ,social or community care in direct contact with service users, with experience working with community organisations that work with people to promote good health and wellbeing.
You will be a good listener, be patient with people and be committed to supporting local communities to care for each other. You should have experience working positively with people facing complex social and emotional challenges. You will have great interpersonal skills in supporting people, community groups and local organisations. You should be motivated to work by yourself as well as work as part of team, supporting other link workers within the social prescription team.
The successful applicant will be subject to a DBS disclosure.
