Keen to join a professional team of Social Care Practitioners working with Sheffield Teaching Hospitals to facilitate the safe discharge of Sheffield residents?
Then this role could be for you. We are looking to recruit a Social Care Practitioner to the Adult Hospital First Contact Service.
This is an exciting opportunity for a person centred individual to join a fast paced Team dedicated to problem solving issues that can be a barrier to someone returning home from hospital.
We are looking for someone who has a good knowledge of prevention services and can offer advice and practical support to people on a wide range of issues from, health, housing, financial and social isolation.
We work with a wide range of organisations so need someone who is able to work with a diverse range of services. We are looking for someone with a good understanding of the principles of Safeguarding and managing risks and the ability to work with people over the age of 18 who have a wide range of issues and disabilities.
You will have knowledge of assessing needs and managing risks of vulnerable adults, supporting people with holistic and person centred plans to achieve their goals. You will also be able to work at pace and in a dynamic way.

Workers are offered regular supervisions and have opportunity for reflective practice.
There are training and development and career progression opportunities within the Service.
Our work base is at the Northern General Hospital, with visits at other Sheffield Teaching Hospital sites, working with people in their own homes and out in the community.

Full-time employees work 37 hours for 52 weeks of the year and we offer a generous holiday entitlement. We are open to discussions about a wide range of flexible working opportunities which benefit you and the Council, including reduced hour contracts, working part time or as a job share.
If you are appointed to this role, your starting salary will be at the bottom of the grade. If you provide payslip evidence that your basic pay is greater than the bottom of the grade, we will consider starting you at the point of minimum advantage within the grade.
