Job Description


Brent Health Matters Team Administrator is crucial to the whole service as it determines the timely response to referrals which guarantees the safe, appropriate management of patients throughout the whole Integrated Care System. The Admin team is an integral part of the Brent Health Matters Team, whose purpose is to streamline and co-ordinate all referrals to the Brent Health Matters Team providing a central base through which referrers can access all disciplines. The Admin team co-ordinates the whole service pathway as well as the monitoring the process. The Administrator is an integral team member and is key to the smooth day-to-day running of the service.

Be responsible for the appropriate and timely referral process of patients to all community agencies, monitoring an integrated partnership between hospital and community colleagues, liaising closely with them to actively and smoothly manage the patients pathway through their hospital episode. The Brent Health Matters Team operates 5 days per week 9.00 -17.00 (time may differ depends on the service needs)
