Job Description

We are looking for a newly qualified or experienced GP to join our dynamic practice with high achievements for QOF and NHSE targets, to work between 4-8 sessions per week

We are located on the borders of Wandsworth and Putney, serving a diverse community of 4700 patients, with new developments and community services being built. We are a team of 6 Gp's, 3 nurses, 1 HCA, and 2 clinical pharmacists and are actively involved in training and development of new recruits. We are part of the Wandle PCN,consisting of 5 practices,and have access to paramedics who do home visits, on site physiotherapist, enhanced access hub, clinical pharmacist,and social Prescriber. The applicant should Key Responsibilities and Duties: Clinical Care: Conducting face-to-face, telephone, video, and online consultations with patients. Assessing patients' health needs, including undifferentiated and undiagnosed problems. Taking patient histories, performing examinations, and making diagnoses. Developing care plans in consultation with patients and in line with current guidelines and protocols (e.g., NICE). Managing acute and chronic conditions. Prescribing medications safely and effectively, adhering to local and national guidelines. Initiating investigations, reviewing results, and making referrals to secondary care or other services. Providing preventative healthcare advice and health education. Managing their own patient caseload and dealing with a wide range of health needs. Participating in the triage of appointments. Undertaking home visits when necessary. Providing cover for other GPs as needed. Managing medical emergencies. Administrative Tasks: Maintaining accurate and up-to-date electronic patient records in accordance with agreed standards (e.g., using systems like SystmOne). Processing repeat prescriptions, dealing with patient queries, and managing paperwork and correspondence in a timely manner. Generating referrals and other necessary documentation. Contributing to the summarization of patient records and ensuring accurate coding. Completing forms, reports, and fit notes. Quality and Governance: Participating in audit activities and contributing to quality improvement initiatives. Complying with practice policies, procedures, and guidelines (e.g., prescribing, confidentiality, data protection, health and safety, infection control). Contributing to the achievement of Quality and Outcomes Framework (QOF) targets and other contractual requirements. Adhering to GMC Good Medical Practice guidelines. Maintaining awareness of and compliance with relevant NHS and local guidelines. Teamwork and Communication: Working collaboratively with other members of the primary healthcare team, including nurses, healthcare assistants, administrative staff, and other allied health professionals. Attending and participating in practice meetings, clinical meetings, and multidisciplinary team (MDT) meetings. Communicating effectively with patients, their families/carers, and other healthcare professionals. Liaising with the administrative team to ensure smooth practice operations. Professional Development: Committing to lifelong learning and continuous professional development (CPD). Participating in annual appraisals and revalidation as required by the GMC. Attending mandatory training sessions. Keeping up-to-date with current medical knowledge and best practices. Potentially supervising and training medical students and junior doctors. Other Responsibilities (depending on the role): Taking on delegated responsibilities for specific areas, such as leading on a particular chronic disease. Participating in service development and business planning activities. Contributing to research activities. Essential Requirements Typically Include: Full GMC registration with a license to practice. Completion of GP specialty training and inclusion on the NHS England GP Performers List. Excellent communication and interpersonal skills. Ability to work effectively independently and as part of a team. Strong clinical skills and the ability to manage a diverse patient population. Good IT skills and familiarity with electronic health records systems. Understanding of NHS policies and procedures. Commitment to providing high-quality patient care. Compliance with mandatory training and safeguarding requirements.

Pay: 74,601.00-82,846.00 per year

Expected hours: 25 - 37.5 per week
