Job Description

To provide and monitor high standards of safe, person centred health care to patients, under the direction of a Registered nurse without

direct supervision by following prescribed care pathways over a 12 hour period.

with all aspects of the delivery of care for this complex group of patients.



You will be required to deliver evidence based care to a high standard. Excellent communication skills and a supportive approach to team working within a rehabilitation setting are key to the success of this service.



An ability to structure your own learning supported by the team gaining new knowledge and skills to benefit yourself and the service will be vital. Some of the skills you can develop may include enteral feeding, tracheostomy management and weaning, and posture and position management. In addition you will be working with families and colleagues in complex discharge planning and best interest discussions.



Excellent organisational and interpersonal skills are essential as is the need to be cheerful and courteous. Good computer skills or the ability to learn will be required for this role as we are using SystmOne to record our patient interventions.



This opportunity is a key role working within a well-established team, working with all disciplines to deliver and achieve the best outcome for the patients who access this service. It is also a real opportunity for someone who is keen to explore their role within rehabilitation and learn and develop some new skills in this area of nursing. In addition, it is an opportunity to establish a therapeutic relationship with your patients and their families.

Develop understanding of all elements of the Nursing process and apply



competency based treatment techniques/care to patients with specific

conditions under the in-direct supervision of a qualified practitioner.

Modify and progress intervention / treatment using own clinical reasoning, notifying a qualified practitioner accordingly.

Monitor the condition and health needs of patients and promptly alert a

Registered Nurse or other Registered health care professionals when there are unexpected changes.

Demonstrate problem solving, and contribute to the solution, working with colleagues.

Make basic judgements on patients' response to prescribed treatment, and report findings to a qualified practitioner.

Plan and prioritise own visits to delegated patients, and ensure these are

documented on SystmOne.



Ensure that SystmOne inputting is completed on a daily basis.

Where there are barriers to understanding, such as hearing impairment, mental capacity impairment and other difficulties in comprehension

explanations , require adjustments in order to gain understanding, consent and concordance.

Establish and maintain therapeutic relationships with patients and carers,

incorporating motivation, encouragement and confidence building to enable patients to engage in their treatment/care/management plan.
