Role: Registered General Nurse



Salary range:

40,000 to 47,000 (dependent on experience). Weekend and evening enhancements and performance related bonus available.

Cheltenham Spa, Gloucestershire, UK

Hours:

Part time and full time positions available (to cover 7 day week Monday to Sunday to include evenings).

Job summary



We are looking for an experienced qualified registered general nurse to provide high quality professional care to our clients attending GHS private clinics. This role involves assessment, treatment and the provision of care across a wide range of health conditions encompassing physical health conditions, mental health illness and chronic diseases. This role is based in the GHS private clinical facility in Cheltenham in addition to providing online consultations. The post holder will report to the GHS Clinics Registered Manager.

Duties



Patient care and assessment:

Evaluate and record a patient's health status including taking vital signs (blood pressure, temperature, pulse, respiration) and physical examination (height, weight, BMI). Capture and record a patient's medical history including past illnesses, hospitalisation, surgeries, allergies, immunisation, medications and lifestyle factors to support effective care co-ordination. Create and implement personalised care plans in collaboration with the patient and multidisciplinary team to support effective care co-ordination. Monitor and record patient's progress, observing responses to treatment and interventions and any changes in health condition. Refine and adapt personalised plans in collaboration with the client and the multi-disciplinary health and care team in response to changes in health condition. In the course of care, assessment and treatment, recognise and address potential challenges e.g. language barriers, cultural lifestyles, medical conditions, learning difficulties, literacy skills and capacity to consent. Provide expert practical and evidence-based advice and education to support the patient and their family across aspects of health and lifestyle, providing emotional support and reassurance. Identify and overcome barriers to change using effective counselling and motivational skills to support adherence to health and care plans. Be a point of contact for the GHS multidisciplinary team involved in the client's health and care, acting as a specialist resource and working closely with add on services. Assimilate and analyse highly complex information from a variety of sources including diagnosis and investigation results. Maintain a working knowledge of legislation, policy and guidance pertaining to mental health and learning disabilities. Take responsibility for ensuring that all clinical practices are in keeping with relevant legislations and policies.

Medication and treatment:



Responsibility for the correct administration of prescribed medication in line with the care plan and the organisational medicines management policy. Dispense and monitor medications as prescribed ensuring patients understand instructions for safe use and are aware of side effects. Perform medical procedures including drawing blood, wound care, administering injections. Operate medical equipment safely and effectively including but not limited to infusion pumps, blood pressure monitors, pulse oximeters.

Communication and collaboration:



Liaise effectively and consistently with the multidisciplinary team to ensure co-ordinated patient care. Be a point of contact for the GHS multidisciplinary team involved in the client's health and care, acting as a specialist resource and working closely with add on services. Maintain detailed, timely and accurate patient documentation including assessment records, care plans and interventions. Communicate assessment and treatment results to the appropriate team members in an appropriate format. Use GHS IT services and data software products in the course and conduct of activities in accordance with GHS policies and procedures. Act as the patient's advocate, representing and communicating their needs and concerns to the multidisciplinary team. Provide appropriate support and engagement with service users, considering their health, social and cultural needs, through a thorough assessment and bespoke planning of their care needs. Develop and maintain bespoke innovative and creative ways of working with clients including those who are difficult to engage. Use a variety of effective communication methods which maximise engagement with clients. Ensure that clients are both involved and consulted about their care at all times, and are part of the decision-making concerning their care. Consider clinical and non-clinical risk management as part of day-to-day work. Complete and communicate incident report forms as appropriate.

Planning and organisation:



Responsibility for organising a daily clinical caseload. Triage and prioritise referrals in collaboration with the multi-disciplinary team. Plan and deliver care with the multi-disciplinary team, taking responsibility for care reviews. Provide leave cover when other members of the nursing team are absent, according to clinical competencies and on an agreed cover rota. Maintain stocks of medical equipment and provisions, resources and literature required for patient care. Responsibility for the safe transport and use of educational tools, IT devices and equipment to support care delivery.

Management, leadership and professional development:



Provide support and guidance to other nurses and administration staff within the service where appropriate. Assist in the induction of new team members. Supervise nurses within the team at a lower grade. Demonstrate good supervision skills in training and developing skills around understanding behaviour; work relationship dynamics; maintaining personal boundaries; work-life balance. Maintain own continuing professional development (CPD). Maintain and develop skills and knowledge to deliver a range of interventions and sets an example of good practice to other team members including skill sharing. Provides teaching / training in the specialist area to the wider workforce. Undertake all mandatory training as required by Global Health Services.

Freedom to act and autonomy:



Freedom to initiate action within clinical / professional policies, accountable for actions. Act at all times as a role model and act ensuring the delivery of high standards of care. Maintain skills and knowledge in relation to clinical area and CPD and Revalidation requirements. Attend meetings relevant to the area of practice and development. Participate in organisational meetings. Keep abreast of relevant political and professional issues in healthcare, particularly in relation to relevant clinical areas.

2. As part of the duty of care to patients, all staff are expected to:

Understand duty to adhere to policies and protocols applicable to infection prevention and control. Comply with key clinical care policies and protocols for prevention and control of infection at all times; this includes compliance with policies for hand hygiene, standards (universal) infection precautions and safe handling and disposal of sharps. All staff should be aware of GHS Infection Control policies and other key clinical policies relevant to their work and how to access them. All staff will be expected to attend prevention and infection control training, teaching and updates as appropriate for their area of work, and be able to provide evidence of this at appraisal.
3. To perform duties to the highest standard with particular regard to effective and efficient use of resources, maintaining quality and contributing to improvements.

4. 7. Ensure you adhere to and work within the local Multiagency safeguarding vulnerable adults policies and procedures.

8. Ensure that you comply with the Mental Capacity Act and its Code of Practice when working with adults who may be unable to make decisions for themselves.
