All about the RolePeterborough City Council Reablement service gives individuals the opportunity, motivation and confidence to relearn or regain some of the skills they may have lost as a consequence of poor health, disability, impairment or accident.We are looking for a new Reablement Practitioner to join our team to support our Reablement support workers working with individuals within our local community.You will work with our clients throughout their journey with our service. Starting with their initial assessment within their home, assessing how we can best support them. You will look at things such as what equipment and occupational therapy aids could best enable them to achieve their independence.Throughout our clients time with our service you will visit them regularly to assess their progress and when their journey with us nears the end, you will use your knowledge and expertise to assess if they need signposting to any other services for further support.As a Reablement Practitioner you will also work closely with our reablement support workers, supporting them and completing supervisions.This role is fast paced and gives the opportunity to truly make a difference to the lives of many in our local community.All about youYou are..
