Qualified Social Worker - Hospital Discharge
Location: Liverpool
Pay Rate: \xc2\xa343.78 per hour (umbrella)
Contract: Temporary, 35 hours per week

We are looking for a dedicated and passionate Qualified Social Worker to assist with Hospital Discharges in the Aintree area.

Position Overview:
As a Hospital Discharge Social Worker, you will play a crucial role in ensuring the smooth and compassionate transition of patients from our hospital to their homes. You will work closely with patients, their families, and our multidisciplinary healthcare team to provide comprehensive care and support during this critical phase of their healthcare journey.

Key Responsibilities:

- Conduct thorough assessments to identify patient and family needs -[
- Develop personalised discharge plans that address patients\' unique circumstances -[
- Collaborate with medical professionals, nursing staff, and community resources to coordinate care -[
- Provide emotional support and counseling to patients and their families -[
- Assist in the arrangement of post-discharge services and follow-up care -[
- Advocate for patients\' rights and access to appropriate services
Qualifications:

- A degree in Social Work (BA or MA) and registration with Social Work England (SWE) -[
- Previous experience in hospital or healthcare social work is preferred -[
- Strong communication, interpersonal, and assessment skills -[
- Empathy, compassion, and a commitment to patient-centered care
