Job Summary

We are seeking a dedicated and compassionate Practice Nurse / Nursing Associate to join our healthcare team. The Practice Nurse or Nursing Associate will play a vital role in delivering high-quality patient care, assisting in the management of chronic diseases, and supporting the overall health needs of our patients. This position requires strong clinical skills, excellent communication abilities, and a commitment to providing patient-centred care.

Duties

Conduct health assessments and develop individualised care plans for patients. Administer vaccinations and injections as per protocols. Monitor and manage chronic conditions, providing education and support to patients. Assist in the planning and implementation of health promotion activities. Maintain accurate patient records and documentation in accordance with IT systems. Collaborate with other healthcare professionals to ensure comprehensive patient care. Communicate effectively with patients, addressing their concerns and queries. Participate in training and development opportunities to enhance nursing skills.

Skills

Registered Nurse qualification with relevant experience in a clinical setting. Proficient IT skills for managing electronic health records and documentation. Strong organisational skills to manage multiple tasks efficiently. Ability to develop and implement effective care plans tailored to individual patient needs. Excellent communication skills, both verbal and written, to interact with patients and colleagues effectively.

Pay: 17.50-19.00 per hour
