The Hospital Discharge Team is responsible for supporting Care Act Assessments in Hospitals and Discharge to Assess Settings for people aged 18 years and over.
The core aim is to support safe and timely discharges with the right treatment, care and support and enable as many people as possible to return to the home they came from.
The team consists of a mix of social workers and social care assessors and there is a close working relationship with health colleagues.
Workers have a hybrid role where there is an expectation to complete assessments in Hospital and Discharge to Assess settings as well as working from home and Shared hub office locations in Winsford and Ellesmere Port. The team is responsible for assessing people across the whole of Cheshire West and Chester.
