Job Description

We are ideally looking for a pharmacist who has completed or near completion of the CPPE pathway. Our preference is a prescriber but can support someone who is starting their prescriber course.
*Non-prescribers will start on 42,500 while they work through the CPPE pathway. Pharmacists who have completed the CPPE pathway and have recently completed the Independent Prescribing course will start on 48,500. Experienced Pharmacists who have been prescribing for 1+ years will start on 50,000.

Edmonton PCN are looking for a proactive pharmacist to join the team at the HUB in Edmonton, London. The appropriate candidate will have the ambition and commitment to provide high quality care to patients, and a passion for clinical development. Working in our HUB alongside a team of Pharmacists, they will be required to act within professional boundaries and provide support in medicines optimisation and to conduct clinical medication reviews.

Job Summary:



To conduct clinical medication reviews and proactively manage complex polypharmacy patients, and reduce inappropriate prescribing where possible

To take on minor ailment clinics

To take responsibility for the management of chronic diseases within the practice

Provide support to practice staff with regards to prescription/medication queries, and to act as a source of medicines information for the practice team (for example, possible alternatives when products are out of stock)

To support the repeat prescription system, and to manage the repeat prescribing reauthorisation process

To complete medicines reconciliation on transfer of care which includes hospital discharge summaries, and to liaise with the patient and community pharmacists to ensure a continuous medication supply

To provide clinical leadership in medicines optimisation and quality improvement and to manage some aspects of the quality and outcomes framework and enhanced services

To support the development and implementation of cost-effective, evidence-based prescribing across the CCG and commissioned services

To develop and implement cost management strategies to ensure financial balance is achieved and maintained

To work closely with and provide highly specialised professional pharmaceutical advice to the PCN, localities, GP member practices, providers and patient

Recognises priorities when problem-solving and identifies deviations from normal pattern and is able to refer to seniors or GPs when appropriate Involve the patient in decisions about prescribed medicines and supporting adherence as per NICE guidelines
To work as part of a multi-disciplinary team, operating from the remote HUB in London.

Person specification



Essential



Completion of a Masters in Pharmacy and registration with the General Pharmaceutical Council

Completion of CPPE

Completion of IP qualification

Have experience and an awareness of common acute and long-term conditions that are likely to be seen in general practice

The ability to follow legal, ethical, professional and organisational policies/procedures and codes of conduct

Excellent interpersonal and relationship building skills

Desirable



Minimum of 1 years post graduate experience in pharmacy

Experience of service delivery within the NHS

Previous experience of working in, or closely within a general practice

Primary Duties and Areas of Responsibility



Undertaking clinical medication reviews, especially with those who are in high risk groups such as: frail elderly, polypharmacy and high risk medication

Reviewing pathology results for patients on regular medication

Provide assistance in completing structured medication reviews and completing targets in relation to the investment and impact fund (IIF) scheme

Signposting where appropriate

To support the improvement of clinical care through the completion of practice based audits

To follow local and national guidelines and formulary recommendations

To ensure education and training is completed- the candidate must enrol on to the CPPE Primary Care Essentials course (18 months)
