Supervision



Regular supervisions on a one-to-one basis including an annual formal appraisal will be held with a line manager. If you are in a patient-facing role, you will also receive regular Clinical Supervisions with a Clinical Supervisor.

Job summary



The PCN Business and Transformation Manager will work with two Primary Care Networks (PCNs) - Horsham Central and Chanctonbury. The post holder will be guiding and supporting each network to strategically develop, and will provide a central source of leadership and management to aid service transformation for the Horsham population. You will be integral to developing the capacity of individuals, teams and services within PCNs, whilst holding line management responsibility for Additional Roles Staff, both clinical and non-clinical, deployed across the PCNs.

The ideal candidate will need to be proactive, have a passion for Primary Care, and have a proven track record in delivering Primary Care improvements. You would support increased access to care for patients on a range of integrated digital transformation projects that would enable the staff to work more effectively and help to sustain the future of general practice services.

As Business and Transformation Manager you would work within the PCN to lead and develop a strategic plan to build a robust infrastructure for PCNs, which:

Is underpinned by a quality improvement methodology Drives integrated working Drives efficiency and reduces duplication Reduces variation between practices Is underpinned with easy-to-understand data packs to support decision-making that includes but is not limited to demand, capacity, utilisation data, staff, and patient experience Builds staff skills and capabilities Enhances patient access whilst protecting our workforce Takes into consideration the digital maturity of the PCN Takes into consideration the population's health needs and health inequalities within our community Improves digital accessibility for our patients
The post holder will build strong working relationships with the Sussex Integrated Care Board, Community Trust, Local Authorities and various Voluntary, Community, and Social Enterprises (VSCEs). They will represent the PCNs and ABC at multiple external partner meetings, serving as a local advocate for Primary Care.

The successful candidate will join a small team, working closely with the Associate Director of Transformation, Head of Practice Support and three other PCN Development Managers that support Mid Sussex, Crawley and Surrey PCNs. The role involves close collaborative working with other ABC teams including; the People Team, Finance, Learning and Development, Communications, Clinical Services and the Refugee and Asylum Seeker Service.

Primary duties and areas of responsibility:



Recruitment, management and support of Additional Roles (ARRS)



Working with close support from the Peoples Team and the Learning and Development Coordinator, the post holder will:

Establish teams of ARRS staff within PCNs, supporting PCNs to identify workforce need and develop or recruit staff in to this Act as an ABC point of contact for a team of clinicians and other care professionals, ensuring they receive the necessary support to thrive in their roles, including objective setting and appraisals with the PCN lead and providing a regular touch point for any concerns Facilitate and lead interviews, onboarding and induction processes for new staff members, ensuring their smooth integration into the PCN and ABC Support or establish where not already in existence networks of ARRS roles of specific disciplines between PCNs Have a good working knowledge of ABC's policies and procedures for managing concerns and issues relating to employee performance and conduct Lead on processes including sickness reviews, investigations, disciplinaries and performance improvement plans, if required

PCN transformation and strategic planning



The post holder will collaborate closely with the Practice Managers and Clinical Directors to:

Develop the PCN's use of services, processes and new technology to improve overall efficiency in a range of projects including but not limited to, cloud telephony, digital triage, promoting the use of the NHS App, and optimising the use of clinical systems Guide and support the PCNs in strategically developing and expanding their networks to better serve the population Design and implement targeted healthcare initiatives, and write bids or proposals to achieve these initiatives Lead and develop a strategic plan that includes a robust digital infrastructure Support PCNs in holding their own mission and plan for delivery, translating this to a business plan against which they can track progress Clarify objectives and priorities for network delivery in line with operational capacity, financial parameters and contract requirements Promote the engagement of all PCN practices and act as an ambassador for the network To proactively build relationships with system leaders to help influence proactive and new models of care To use business intelligence tools to aid data-driven decisions Attend PCN board meetings and Practice Manager meetings Work closely with ABC Finance team to develop and maintain a good understanding of the PCN's financial position

Business Operations



To provide oversight and accountability to the delivery of PCN contracts and specifications Identify and record risks and issues and develop contingency plans with Network Leads Ensure all relevant national strategies, policies, targets, and frameworks are fully implemented To drive issues through to resolution, acting as a point of expertise To support the collation of financial information, compiling and presenting financial reports To ensure that the PCN's physical and financial resources are managed and maintained for maximum effectiveness To foster a culture of collaboration and promote learning to drive continuous improvement To take an active role in the development and embedding of the PCN's culture, values and reputation To represent the network at external meetings To facilitate progression for the PCN staff to promote retention

Ensuring delivery of the PCN DES



The post holder will:

Build a working knowledge of relevant NHS contracts and initiatives and understand the impact or opportunities for GP practices and their population Support PCN's by providing straightforward and simple processes to follow to demonstrate DES compliance and achieve financially incentivised targets Share learning and best practice between PCNs in achieving full delivery Offer direct support to the PCN Clinical Directors so that their role, as outlined in the Network Contract DES, can be fulfilled

Integrated Neighbourhood Teams (INTs)



The post holder will be responsible for building and maintaining key working relationships and for driving the development of Integrated Community Teams by:

Working closely with the Horsham Neighbourhood Lead in the development of Integrated Community Teams Represent the PCN's and ABC at internal and external partner meetings including neighbourhood workshops To operate effectively, the post holder will need to establish excellent working relationships with:
- GPs (partners and salaried) across the PCN

- PCN Practice Managers and Deputies

- PCN employed Additional Roles staff

- ABC wider team e.g. Clinical Services, People Team etc

- Sussex Integrated Care Board

- Community Care: e.g. SCFT AND SPFT

- Third Sector: e.g. Age UK

- Local authorities e.g. District Council

Project management



The post holder will:

Drive improvements in patient access, service, and experience Ensure that digital and operational transformation is embedded and aligned with system and national strategies in collaboration with the ICB Digital Transformation team Manage the set up and delivery of multiple projects working with colleagues and experts to scope, plan, design and oversee effective delivery Provide expertise of best practice change and project management methodologies Interrogate and analyse data to assess demand, capacity, and activity to support the delivery of clear project reporting and project outcomes Submit regular high-level project reports showing progress against milestones, status, resource requirements, risks and dependencies and recommend a range of options Develop key performance indicators and milestones for specific programmes and projects, managing conflicts and ensuring interdependencies are managed effectively Create and maintain accurate documentation in relation to finances, standard operating procedures, contracts, contacts, and decision-making, which is accessible centrally Hit government set targets and support population health management Identify opportunities for streamlining the service within the PCN Enable PCN staff to maximise clinical efficiency and enhance internal and external communications Embed patient and public involvement at all levels of decision making Communication Engage and develop effective communication channels with key strategic policy makers to influence policy making Communicate the vision for the PCN and support the development of strategy and operational policies to support this vision Act as a champion for patients and their interests and involve the public in the policy development and decision-making Build relationships and facilitate cooperation between the practices and at neighbourhood level to improve patient care delivery
