JOB TITLE: Patient Support Team Member



REPORTS TO: Patient Support Team Lead

Essential - GCSE grade A to C in English and Maths IT Literate Administrative Qualification/Experience



Desirable - Experience of working for a GP surgery



The successful candidates must be able to work 18hrs Monday - Friday (Over 3 days)



There is 2 part time position available



Job summary:



We are seeking an enthusiastic and well-organised General Practice Assistant to work within Abbey Road Surgery. You will support the practice GP's, nursing and administrative teams in the delivery of clinical and administrative services, working as part of the practice multi-disciplinary team, delivering care within their scope of practice to the entitled patient population.

Working as part of a well-established team. Receive, assist and direct patients in accessing the appropriate service or healthcare professional in a courteous, efficient and effective way.

Provide general assistance to the practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone

Key requirements of the none-clinical role

Process and effectively signpost patients to the appropriate healthcare professional, depending on the presenting condition using the practice's signposting tool. Process patient requests for appointments, telephone calls, information requests following the practice policies and procedures. Initiate contact with and respond to requests from patients, team members and external agencies Enter read/SNOMED CT code data inputting data into the patients' healthcare records as necessary Carry out system searches as requested Participate in practice audit as directed by the audit lead Support all clinical staff with general tasks as requested Process repeat prescription requests Support achievement of QOF and National/Local Enhanced indicators Administrate annual reviews recalls and ad-hoc recalls Good timekeeping and ability to meet deadlines Contribute to public health campaigns (e.g., flu clinics) through advice or direct care Have a thorough knowledge of the surgeries protocol always adhere to them Ensure building security - have thorough knowledge of doors/windows/alarm Undertake all mandatory training and induction programmes Attend a formal appraisal with their manager at least every 12 months. Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreed Communicate information clearly to a variety of groups, including healthcare professionals and patients Have good technological skills Be willing to learn new skills and adapt to GP needs
Training and Education

Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work Attend all relevant annual updates Inform the lead GP of any concerns regarding GPA role and any professional development needed Be aware of own professional boundaries and what to do when you have reached them

They may also have access to information relating to the Practice as a business organisation. All such information from any source is to be regarded as strictly confidential Information relating to patients, carers, colleagues, other healthcare workers or the business of the Practice may only be divulged to authorised persons in accordance with the Practice policies and procedures relating to confidentiality and the protection of personal and sensitive data

The post holder will:
