Occupational Therapist x 3 (Hospital Transfer of Care Hubs) Job description
Permanent, Full or Part Time hours will be considered.
This is an opportunity to be part of an exciting new service within Bristol City Council adult social care. At both hospital trusts within Bristol, we are creating two new Transfer of Care Hubs to support our citizens with discharge planning.
The Transfer of Care Hubs will bring us together with our acute and community health partners, alongside Voluntary Sector colleagues, to create new multidisciplinary teams. You would be integral in providing an excellent service to our citizens easing their transition home following a hospital admission.
The role of an Occupational Therapist in social care is varied and practitioners build skills across a range of disciplines working with us. We welcome applications from newly qualified and experienced practitioners alike with the opportunity to follow career progression pathways into advanced grade or management roles.
You will have a full induction, supervision from experienced and very supportive colleagues, and opportunities for training and continuous professional development activities. We endeavour to ensure that everyone has the opportunity to develop highly skilled, evidence-based practice, utilising the 4 pillars of the RCOT career development framework. We provide regular OT specific learning and development activities and also provide practice educators with a generous payment for supporting student placements.
