Occupational Therapist - Band 6

PT 22.5 hrs a week Monday - Friday

Hourly rate of 19.02 to 22.90 - Depending on experience

Rowans Hospice is a charity that provides free care and support to adults and their families living in the Portsmouth and South East Hampshire region who have a life-limiting illness. Established in 1994, our income is generated by ongoing support of the local community, donations, legacies, fundraising and through our retail shops.

Many people think that hospices are all about dying, but through our many different services, we support people to live as well as they can with their illness. From the moment of diagnosis through to bereavement support. Our Living Well Services put an emphasis on living with a life-limiting illness and patients and carers can drop in and gain advice and support to maintain individuals' resilience.

The Hospice is also supported by an assembly of volunteers, from various backgrounds and ages who are crucial in supporting the teams to ensure people live well to the end of their life and attain a good death within a caring and compassionate environment.

Your Role

The Occupational Therapist role will work across the Family Services Department which is led by the Lead Social Worker who is responsible to the Clinical Director. The role is based at Rowans Hospice, Purbrook.

Job Purpose

To work as part of the Therapy team to deliver a high-quality occupational therapy service, to patients with specialist palliative and supportive care needs. To assess, plan and implement programmes of care and intervention and support discharge. You will extend this service to patients, families, and carers within the In-Patient Unit (IPU), Living Well Service (LWS), and in the community, as required to meet the needs of the patients and the service..

Key Areas of Responsibility / Key Accountabilities

To delivery high quality safe and effective care to patients, their loved ones and relatives.

To provide holistic OT assessment of patients' needs I all RH settings.

Therapeutic handling of patients for assessment of functional activities, such as personal care, toileting, dealing with in continence and handling of body fluids, wound fluids, hand hygiene.

Implementing plans and programmes that have been agreed with the patient, and where possible, their main carer

Review and evaluate outcomes of interventions and adapt as needed.

Attend and participate in the morning IPU handover.

Attend and participate in the weekly IPU MDT

Plan, organise and prioritise own caseload whilst assisting in managing the priorities of others in the team, such as the Therapy Assistant and students.

Demonstrate patient advocacy through all aspects of care

Ensure accurate and timely documentation to support Occupational Therapy intervention and actions in line with professional standards, local policies and procedures.

To work closely with the Therapy team to support the highest standards of care is delivered.

Completion of access visits to assess home environment, prior to discharge planning from the Inpatient Unit.

Outcomes of any asse3ssment must be agreed with the patient and their main carer and documented clearly in-patient record.

Support the discharge of patients from the IPU including access visits.

Accept responsibility for the patient within their own home and when lone

working.

Prescription of ADL equipment, giving advice, instruction, and visual documentation on its safe use.

Explain the use of and teach moving and handling techniques, with equipment to patients, relatives and carers.

Develop and maintain effective relationship with local equipment services.

Person Specification

Experience, knowledge and qualifications

Degree or Diploma in Occupational Therapy

HCPC registration

Evidence of continued professional development, including reflective practice, in service training courses attended, participation in clinical governance

Post graduate courses relevant to the speciality

Member of special interest group

Good listening skills

IT literate

Essential Competencies

Experience of working with people who have life limiting illnesses.

Working with people who have complex physical and psychological needs

Working in a multi-professional team.

Experience of staff/student supervision

Experience of working with patients/families in the community.

Familiar with a fitting a range of adaptive equipment, standard and specialist

Evidence of excellent verbal and written communication skills

Ability to deal with complex patient needs and in stressful situations

Flexible and adaptable

Able to recognise own limitations

Desirable Competencies

Experience of working with a range of conditions seen in palliative care, such as motor neurone disease, types of cancer, respiratory, coronary disease etc

Experience of individual or group outpatient work.

Accessing equipment from equipment services

Full clean driving licence.

Other

Undertake any duties, which may be reasonably required within the scope of the role.
