The MND Care Coordinator role is responsible for co-ordination of care and support to people living with MND across Powys.The post will be based in Powys, location to be agreed. Applications are welcomed from professionals registered with any health or social care professional body, such as nursing, social work or the allied health professions.If you have the relevant qualifications, experience, knowledge and skills, together with a strong desire to improve care for people with MND and their families, then we would love to hear from you.The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.Main duties of the jobhe post holder will be expected to:Deliver local support to patients diagnosed with MND in Powys (via a single point of contact).Track patients along their disease pathway from diagnosis to end of life care.Coordinate the local multidisciplinary care of patients living with MND across Powys Teaching Health Board area in primary and secondary and third sector care settings ensuring patients can access high quality care that adheres to the MNDA standards of care and best practice guidance.Liaise with the Neurologists and the MND Care centre in Morriston Hospital, Nevil Hall Hospital, Walton Hospital Liverpool for specialist input of MND care.Be a non-urgent single point of contact for patients with MND, their carers and professionals as a resource for information advice and signpostingEvaluate and audit the Care Coordinator role as agreed with the MND Care Co-ordinator Steering Group.About usBeing the smallest Heath Board in Wales means that you won't get lost in the crowd.
