Surrey County Council is seeking a dedicated and compassionate Mental Health Social Worker to join our dynamic Adults Services team in Reigate. This role offers the opportunity to work with a multidisciplinary team to support individuals with mental health needs, ensuring they receive the appropriate care, support, and intervention.

As a Mental Health Social Worker, you will be responsible for conducting assessments, developing care plans, and providing ongoing support to individuals with complex mental health needs. You will help individuals live as independently and fulfilling lives as possible, promoting social inclusion and empowering them to access the resources they need.

Key Responsibilities:

Conduct comprehensive assessments of individuals with mental health conditions, evaluating their physical, emotional, and social needs. Develop, implement, and review care plans in collaboration with service users and their families. Support individuals in accessing relevant mental health services, resources, and interventions. Provide case management and advocacy to ensure individuals receive necessary services and support. Coordinate care and services by liaising with healthcare providers, housing, and community agencies. Participate in team meetings, supervision, and continuing professional development activities. Maintain accurate and timely records of assessments, care plans, and interactions with service users in line with safeguarding protocols and confidentiality guidelines. Provide expert social work support in crisis situations, ensuring effective risk management and decision-making. Promote social inclusion by guiding individuals on housing, employment, and social integration. Ensure all interventions are culturally sensitive and tailored to the individual's unique needs.

Essential Criteria:

Qualification:

Recognised Social Work qualification (e.g., BA or MA in Social Work).

SWE Registration

Experience:

Experience working with adults with mental health issues, ideally in a community or clinical setting.

Knowledge:

Knowledge of relevant mental health legislation, including the Mental Capacity Act 2005 and safeguarding procedures.

Skills:

Excellent communication, assessment, and case management skills.

Personal Qualities:

Compassionate, resilient, and committed to working with vulnerable individuals. Ability to work independently and as part of a multidisciplinary team.

Car Driver:

A full UK driving license and access to a vehicle is required for this role.
