Main duties of the job



The Medicine Care Coordinator will work with the Lead Pharmacist and PCN Manager to achieve KPIs, and exceeding key targets and performance indicators outlined in the SOPs and within the business plan.

The role is not a regular 9am-5pm job; the job requires innovation, flexibility and commitment and the postholder will be required to work resourcefully as part of the team to ensure tasks are completed.

To provide an accurate, timely and high quality service to Practices and patients handling acute and repeat prescription requests for registered patients.

2. Key Duties & Responsibilities:



To ensure that all medicine and appliance prescription requests are checked against the patient records and whenever there is doubt about the appropriateness of the item, or about the dose or labelling instructions, to check with the appropriate prescribing clinician/ pharmacist.

To ensure that all systems by which prescription requests are received by the Practice, including by paper, email and online, are monitored to ensure no requests are overlooked.

To respond, under duty pharmacist/ clinician supervision and instruction, to requests for acute prescriptions.

To generate repeat prescription requests received through different modes, including written requests from patients, letter and online within 48 hours of request. To generate patient requests for new prescription items not on their repeats list by discussing with the duty pharmacist/ tasking the appropriate clinician. To check for medication reviews/blood tests/BP/ relevant monitoring needed before issuing prescriptions and take appropriate action. To ensure correct protocols are followed regarding the ordering and tracking of controlled drug requests.
To monitor high risk drug prescriptions and paperwork to ensure compliance with protocols.

To prepare and print repeat prescriptions. To ensure prescriptions are provided to clinicians in a timely manner for sign-off and identify and escalate delays. To generate prescriptions for injectable medications administered by clinicians for PPA claims purposes.
To liaise with Practice Pharmacists, Community Pharmacies and other relevant outside organisations to resolve problems with prescriptions quickly with minimal disruption to patients.

To deal with general enquiries to and from patients regarding their medication.

To update patients' prescribing records in accordance with discharge information received from other providers e.g., hospital outpatient letters.

To identify potential over-ordering/over-use of medication and flag up with the duty pharmacist/ clinician.

To communicate and liaise with Practice Pharmacist teams and Medicines Management teams as required.

To undertake any other duty as requested by prescribing clinicians, Patient Services Manager or Area Manager.

To attend meetings as required and participate in PLT sessions.

To assist with training on prescription administration for new and existing staff To ensure learning from all individually attended training sessions is cascaded to all members of the team. To check and respond regularly to tasks, emails and notifications. To ensure drug fridge monitoring is undertaken. To always maintain patient confidentiality. To work collaboratively with Pharmacists and prescribing clinicians to support service delivery. To participate in extended hours, work as required by the organisation.

What are the minimum requirements all candidates need to meet?

Knowledge



Educated to NVQ3 or above

Good understanding of the General Practice prescribing environment including interaction with other NHS organisations and third parties.

Understanding of how General Practice relates to the wider healthcare system.

Aware of the importance of innovation in healthcare and the evolving landscape of primary care.

Skills



Proficient in MS applications including Outlook, Excel and Word and other related software packages.

Ability to lead, motivate and inspire team members.

Good written and verbal communication skills.

Excellent team working and customer service skills.

Able to deal with difficult situations and challenging behaviour.

Ability to coach, mentor and facilitate learning.
