Our critical time intervention service supports vulnerable people who are homeless and residing in temporary accommodation with multiple co-occurring needs to stabilise their lifestyle maintain their temporary accommodation and successfully obtain permeant accommodation. The service delivers translational support encourage independence in their. We can offer employment within an established team that employs a non-judgemental approach, sensitivity and understanding.

You will have your own caseload, maintaining oversight of the people you support and their progress from the point of service entry through to service exit and independence.

Who are we looking for?
A passionate, energetic and inspirational Support Worker who will work within our team across the county of Conwy to provide high standards of person-centred support using housing first principles. Our service is for people aged 18 and over some with complex needs including social, emotional and behavioural difficulties. Through a creative and caring approach, you will build good, trusting relationships, supporting people to achieve their aspirations. This is a fast paced, reactive environment where energy and initiative are vital.
We are looking for someone with a sound understanding of the support needs of people facing homelessness who typically use our services.
We are looking for an individual with great communication and time-management skills, who possesses the ability to remain calm in stressful situations, be flexible and open to change, and work collaboratively with others.
The ideal candidate should have experience of supporting people to obtain accommodation, experience of person centred planning and working with people with multiple, co-occurring high level needs Additionally, they will hold a good understanding of the housing first principles.
Duties & Responsibilities include but are not limited to:
; Provide an enabling, personal centred, collaborative and strong partnership approach, working closely with other organisations within the community to maximize opportunities.
Work with people to help them articulate their goals, dreams and aspirations offering personal, beneficial and emotional support. The work will be informed by core Housing First principles.
Regularly review the person centred support plan to update, record and monitor progress of each person.
Liaise with Social Landlords, Private Landlords and Homeless prevention team in Conwy to assist with the person to access accommodation of their choice, including practical help with furniture, benefits advice, accompanied visits (where possible) and liaison with landlords.
Develop and maintain relationships with people to enable person-centred support through regular contact including visits to their home and appropriate community settings.
Provide support to help people manage their own lives and their home to their full potential including dealing with benefits, budgeting and neighbour relations
An Enhanced DBS check will be required for this role.
The ability to communicate in Welsh is desirable but not essential.
A driving license and access to your own vehicle is essential.
