Are you a Qualified Social Worker with experience in Adults Hospital Social Work?

A local authority based in the Northwest (Merseyside area) is giving you the opportunity to join their busy and enthusiastic workforce as part of their Hospital Discharge Team.

You will be working closely with hospital staff in a Multi-Agency Service consisting of nurses, OT, Drs, and support workers to help support patients being discharged to either home or intermediary care. The post will focus on ensuring patients have the correct package of care necessary for them to safely be discharged.

Responsibilities include:

• Completing Social Care Assessments for residents within hospital
• Managing fast-paced work and a caseload

• The social worker will need to complete Care Act assessments and long-term support plans, Mental capacity assessments, and Best Interest Assessments
• Identifying appropriate support for discharge voluntary services; 24 hour placement or domiciliary care package

• Reviewing support following discharge

Managers are willing to consider a 4-day condensed week, but you will be required to complete face-to-face assessments and you will often be based in either Whiston or Warrington hospital on working days. The post offers a greater deal of autonomy as you will have the opportunity to manage your own diary, giving you an excellent work-life balance.

The ideal candidate will be:

• Social Work England registered
• Enhanced DBS Checked
• In possession of a Full UK drivers licence
• Available to start ASAP
• Experienced in Hospital Social Work
