Modality Partnership East Surrey Division has a part-time/ full-time vacancy (25 hours to 37.5 hours per week) for a Health Care Assistant. The postholder will be a high performing HCA working at Birchwood Medical Practice/Smallfield Surgery. We are seeking dedicated postholders with primary care experience who will work collaboratively with the general practice team to meet the needs of our patients through the delivery of safe patient care.
The post holder will support as part of the wider nursing team to promote the health and wellbeing of our patients by assessing their needs, providing pre-screening activities and health care for chronic diseases, health education advice and support, implementing requirements in line with patient requirements, community, or other settings, maintaining clinically accurate records during clinics.
This job is suitable for individuals who can demonstrate safe and effective patient interactions ensuring the delivery of high-quality patient care whilst working alongside the wider nursing team and practice team. Salary for this role is 12.50-14.50 per hour.

Main duties of the job
This is a hands-on patient facing role; we are seeking individuals to join our team who are caring, compassionate and confident, who demonstrate effective communication, excellent customer service skills, good IT skills and the ability to support patients with interventions and/or queries. The postholder will be a key member of the practice team, responsible for undertaking patient health checks inc. BP / ECG monitoring / giving of vaccinations/injections (such as B12, flu and pneumococcal), taking blood samples, health and wellbeing advice and promotion.

Communicate courteously and effectively with patients, relatives and careers.
Report all matters pertaining to patient care - changes in condition/circumstances to a relevant member of the nursing or medical team as a matter of priority and ensure that a document record is kept.
Ensure that the clinical rooms at all times kept in a clean and orderly manner and that relevant equipment is ready for use by nursing and medical staff during clinic sessions.
Assist in clinic sessions as directed by members of the nursing and medical staff ensuring that all equipment is clean and sterilised following clinic sessions.
Ensure that the drug refrigerator is at correct temperature on a daily basis and that drugs are within their expiry dates and used in correct sequence. Order drugs and other supplies as directed ensuring that adequate levels of stock are available in the surgery.
Record fridge temperatures.
As directed by the Practice Nurses working within local policy/protocols undertake and develop skills in phlebotomy, 24 hour ABPM, ECG recording, advanced phlebotomy, spirometry/peak flow, chaperoning duties, basic wound care, ear syringing, B12/flu/pneumococcal vaccinations, Long Term Conditions annual initial assessments.
Undertake NHS health checks new patient health checks and general health checks.
Undertake a range of diagnostic procedures e.g. urinalysis, blood pressure monitoring, pregnancy tests, diabetic foot checks, (within capabilities and as required for the role).
Delivery of health promotion e.g. smoking reduction advice, diet, exercise and alcohol.
Maintain stocks control for all clinical rooms.
Support practitioners to carry out healthcare activities.
Act at all times with regard to the confidentiality and sensitivity of issues to do with patients, their care and treatment.
Record and document all patient contact.
Carry out relevant clerical and reception duties as required in relation to nursing services.
Assist in audits as required e.g. note sampling, data input and information gathering.
Attend relevant meetings as required.
Have knowledge of and act at all times within the boundaries of Health and Safety legislation and data protection acts.
Assist in the call and recall systems in relation to: chronic disease management; immunisation, cervical cytology screening.

Qualifications and Experience:
GCSE Grade A-C Maths and English (Or equivalent)
NVQ 3 or equivalent experience in Health Care Assistant roles
Knowledge and experience
