The team accepts referrals for individuals living in the community, care home residents and those currently in hospital. Areas of social work you will encounter on our team include but are not limited to multi-disciplinary working with families, unpaid carers, third sector and health professionals; assistive technology; supporting unpaid carers; reablement; direct payments; working with those living with Dementia diagnoses; hospital discharge and hospital avoidance; safeguarding; self-neglect; Mental Capacity and Best Interest decision-making; Continuing Healthcare assessments; intervention under the Mental Health Act; and Court of Protection.
