Partnership approach

We support a child centered whole family approach - helping to embed the culture of understanding the needs of the whole family, not just the person requesting support and making sure the choice of interventions is given. Also trying to ensure that parents are communicated with regularly, clearly and that they navigate the system quickly and successfully.

Main purpose of job * To work in partnership with clinicians in AAT (access and advice) to ensure the smooth transition of cases held on system one to partner agencies. Assessing cases and ensuring the needs of the children, young people and their family\'s needs can be met by partners intervention.
  • Make discovery phone call to parents/cares and young people (if suitable) to assess the current needs of the children and young person, then writing up clear and accurate forms to allocate them to the appropriate services. Ensuring the NHS database is kept up to date.
Key Responsibilities
  • Assess routine cases held on the AAT database (system one), review the needs of CYP and refer them to programmes or partners related to their areas of need
  • Assess the needs of young people through telephone communication with parents/carers
  • Where suitable, ensure that the young person is involved in the assessment and planning of their referral
  • Attend daily multi agencies meetings with clinicians within AAT to discuss the needs of the referrals and agree the correct allocation
  • Ensure information related to the management and assessment of referrals is recorded accurately in line with Mindworks and The Eikon Charity policy
  • Build positive relationships with staff from AAT and represent SWP at AAT
  • Have a knowledge and understanding of the services SWP partners offer
  • Keep up to date records for the purpose of monthly reports
  • Build positive relationships with young people and where necessary parents and carers
  • To attend and participate in any professional training as may be identified
  • Work within Eikon\'s equal opportunities, health & safety and safeguarding policies at all times
  • Ensure processes are in place to safeguard the transition of cases
  • Complete appropriate assessment reports for both Eikon, SWP and AAT
  • Discuss with AAT Discovery Coordinator when issues arise with cases
  • Understand emerging need within AAT and liaise with AAT Discovery Coordinator
  • Record on AAT database in line with the needs of the contract and GDPR
  • Ensure information related to the management and assessment of referrals is recorded accurately in line with Mindworks and The Eikon Charity policy
