Transition Worker Grade 7 Term Time only 37 Hours per Week \xc2\xa325264 - \xc2\xa328302 Ancora House School Ancora House School and its governors wish to recruit an outstanding practitioner to join our talented and experienced team. Ancora House School has two sites; Ancora House Adolescent Psychiatric Unit, on the Countess of Chester Hospital Site and our Ancora House community provision at Castle Park, Frodsham. The vacancy has arisen in our SEND and Transition Team; and is predominantly based at our community site in Frodsham however there is an expectation that all staff work flexibly across sites as required. The role is challenging, diverse and rewarding; the successful applicant will bring passion, commitment and fantastic organisational skills to the team. During and following admissions, the Transition Worker is responsible for gathering information to support the Transition Plans for young people who are referred to us ; they will contribute valuable information to the wider team to inform the plans and to ensure any barriers are identified and addressed in a timely way. Strong skills in communication with, and advocacy for vulnerable young people are essential with the ability to deliver sessions to small groups or on a 1:1 basis. A positive and solution focused approach will encourage young people experiencing significant mental health, social and emotional challenges to regain aspirations, consider their future plans with more optimism and to engage with education, employment or training. Sound safeguarding and thorough information sharing will ensure destination settings feel confident and skilled to support the sustained transition of young people following their discharge from hospital.
