Southend Council are looking for an experienced Adults Social Worker to join this community team.

This role is for 30 hours per week.

As a Community Social Worker, you will play a vital role in providing support, advocacy, and intervention services to individuals and families within the community. You will work collaboratively with clients, community organisations, and other stakeholders to address social issues, promote well-being, and empower individuals to overcome challenges and achieve their goals.

Key Responsibilities:

Assessment and Case Management: Conduct comprehensive assessments of clients strengths, needs, and circumstances to develop individualised care plans and interventions. Provide ongoing case management and support to clients, connecting them with appropriate resources, services, and benefits to address their needs.

Crisis Intervention: Respond to crises and emergencies within the community, providing immediate support, assessment, and intervention to individuals and families in distress. Collaborate with emergency services, mental health professionals, and other agencies to ensure the safety and well-being of clients.

Counselling and Advocacy: Offer counselling, emotional support, and advocacy to clients facing social, emotional, or psychological challenges. Empower clients to voice their needs, rights, and preferences, and advocate on their behalf to access services, entitlements, and opportunities.

Community Engagement: Build positive relationships with community members, stakeholders, and organisations to identify needs, resources, and opportunities for collaboration. Organise and facilitate community events, workshops, and support groups to promote social connectedness, education, and empowerment.

Prevention and Education: Develop and deliver educational programs, presentations, and outreach initiatives to raise awareness of social issues, promote healthy lifestyles, and prevent social problems such as substance abuse, domestic violence, and homelessness. Provide training to community members and professionals on relevant topics.

Networking and Collaboration: Collaborate with local agencies, government departments, schools, health care providers, and nonprofit organisation to coordinate services, share information, and advocate for systemic changes that benefit the community. Participate in multidisciplinary teams, committees, and task forces to address complex social issues.

Documentation and Reporting: Maintain accurate and up-to-date records of client interactions, assessments, interventions, and progress notes in accordance with agency policies and legal requirements. Prepare written reports, case summaries, and documentation for meetings, reviews, and funding applications as necessary.

Qualifications and Requirements:

Bachelors or Masters degree in Social Work or a related field.
Registered with the relevant regulatory body (e.g., Social Work England).
Previous experience in community-based social work or related roles, with knowledge of social issues, community resources, and intervention strategies.
Strong assessment, communication, and interpersonal skills, with the ability to engage and build rapport with diverse individuals and groups.
Ability to work independently and collaboratively within a multidisciplinary team, demonstrating flexibility, resilience, and cultural competence.
Proficiency in driving and access to a reliable vehicle for travel within the community.
