An exciting opportunity has arisen for a Community Care Officer in the Community Adult Learning Disability Team, specialising in Local Supported Employment.

The role is to coordinate a Local Supported Employment Initiative for people with a Learning Disability and/or Autism. The role will include direct support and assessment in relation to people with learning disabilities and/or Austim, being able to move into employment.

The successful candidate will be need to develop positive relationships with colleagues, partners and local employers.

We are looking for someone who has is creative, adaptable and has the know how to make a difference in supporting people in sustainable employment.

The post will report directly to the CALDT Team Leader.

To provide a high quality, integrated, effective social work service to adults with a diagnosed learning disability and/or autism, carers and the community in accordance with the Health and Social Care Act 2014 statutory requirements, local policy, procedure and guidance.

The post holder will need to be able to work autonomously using initiative to manage their own caseload with the oversight and supervison of the Advanced Practitioner. The role will involve undertaking holistic, strengths based assessments with adults, carers and families to help them make informed choices and decisions, enabling them to clarify and express their needs, manage risk and contribute to service planning. You will provide written and verbal records and reports which are concise, factual and informative based on analysis of evidence. You will recieve on-going training throughout both formally and informally.
