Job Summary

We are seeking a dedicated and experienced Prescribing Clinical Pharmacist to join our healthcare team. The ideal candidate will play a crucial role in ensuring the safe and effective use of medications for patients, particularly in critical care settings. This position involves collaborating with healthcare professionals to optimise patient outcomes through medication management and patient education.

Duties

Review and interpret medication orders to ensure accuracy and appropriateness for patient care. Provide expert advice on medication administration, including dosage adjustments and potential drug interactions. Conduct thorough assessments of patients' medication regimens, particularly for those in the Intensive Care Unit (ICU). Collaborate with physicians, nurses, and other healthcare staff to develop comprehensive care plans. Educate patients and their families about medications, including proper usage, side effects, and potential interactions. Monitor patient responses to medications and make recommendations for therapy modifications as necessary. Maintain accurate records of medication therapy management and participate in quality improvement initiatives.

Skills

Proficient in medication administration techniques and protocols. Strong knowledge of human anatomy and physiology as it relates to pharmacology. Excellent patient care skills with a focus on empathy and communication. Experience working in an ICU setting is highly desirable. Ability to work collaboratively within a multidisciplinary team environment. Strong analytical skills for assessing medication efficacy and safety. Commitment to ongoing professional development and staying current with pharmaceutical advancements.

Pay: 44,454.00-49,441.00 per year
