Main Responsibilities:
Efficient Referral Management: Accept and accurately register patient details from primary care clinicians, ensuring seamless integration into our clinical database.
Triage Excellence: Timely triage of patient referrals with accurate entry of outcomes on our clinical system, ensuring swift response to patient needs.
Effective Liaison: Collaborate with GP Surgeries and referring organizations to gather precise patient information and facilitate seamless care coordination.
Appointment Coordination: Work closely with the Service Manager and location managers to ensure sufficient appointments are available, booking appointments as directed.
Data Accuracy: Enter and ensure documentation of all clinical information is correct, maintaining high standards of literacy in all communications.
Administrative Support: Perform general administration duties as needed, maintaining professionalism and efficiency at all times.
Communication Mastery: Answer telephone calls promptly and professionally, handling queries with tact and efficiency.
Discharge Letter Preparation: Create accurate discharge letters and ensure timely dispatch at the end of each day.
Professional Development: Participate in training courses and annual appraisals to enhance expertise and knowledge.
General Requirements:
Record work activities in compliance with local policies and standards.
Foster professional relationships with team members and external stakeholders.
Uphold principles of equality, diversity, and confidentiality in all interactions.
