Job Description
The successful applicant is expected to have relevant proven experience of chronic disease management in the NHS, within a GP Surgery environment including QOF and Emis web experience. The candidate needs to provide routine long-term condition management for asthma/COPD (including spirometry) and diabetes (including medication changes).
