About the role
The successful candidate will provide direct support and advice to clients aged between 0-18 who have a range of complex and challenging needs. As well as supporting the client and their carers and families you will also support the work of the whole team.
Responsibilities
:
Support/organise or provide practical support to clients in & outside their homes.
Support clients to ensure their views and wishes are heard when completing care reviews and assessments through visits at home, school and respite.
Plan and implement interventions and actions for your allocated cases and or support colleagues to do so for their allocated cases.
Monitor, support and review need, and services provided for clients, their carers/families.
Undertake Review meetings, take minutes and update plans with clients and other professionals.
Liaise with wider multi-disciplinary teams to request support and ensure care needs are being met.
Ensure the Safeguarding of clients with safety plans or view to refer for additional support and intervention where necessary.
Conduct yearly Child & Family assessments of clients' circumstances and issues to determine intervention.
To maintain high standard documented and accurate case files, records, and client information, in the correct format and location and in compliance with legislation and best practice.
Assist in more complex cases under supervision, or where appropriate shadowing experienced colleagues.
Undertake duty requirements in the team.
Undertake EHCP's review paperwork.
Mobile:
A 'mobile' role gives you the flexibility to schedule your own diary, so you can meet with colleagues and clients in a variety of locations. You'll access digital folders and resources on your laptop and be able to use the network wherever you are based. Working from home will be common, as well as meeting your team in the office where necessary.

About you
You will hold a relevant qualification, such as NVQ 3 / HNC or equivalent. Have appropriate experience of working with the relevant client group and understanding of the relevant statutory frameworks / requirements and practical knowledge of a range of procedures and interventions to support clients.
You will have sensitivity and empathy to build trusting and supportive relationships with clients and families and skills to influence, persuade and motivate clients to achieve agreed goals and targets.
