What are we looking for?
Cleveland Clinic London are looking to recruit a Case Manager to join our Case Management Team, with a competitive salary range on offer, up to \xc2\xa344,000.
This is a Monday to Friday days-only role, generally between 0900 \xe2\x80\x93 1700, with flexibility as per the service requirements.
The Case Manager\xe2\x80\x99s role is a key on the delivery of high quality, cost effective and innovative nursing practice consistent with regulatory and professional requirements. Acting as a visible credible role model, you will create a patient-centred environment within the CCL putting the patient first.
The Case Manager will support the Lead Case manager in delivering the Cleveland Clinic London (CCL) key performance targets in relation to quality, patient experience and cost effective service delivery.
What will your duties include?
Works collaboratively with multidisciplinary teams, Institute, Unit managers, Business revenue- Finance, Global patient Services, External stakeholders, CCL caregivers and all third party payers to maintain active communication in order to provide cost effective, timely, safe and appropriate patient management.
Works in partnership within the Global Patient services and Medical Legal teams to proactively identify and resolve delays and barriers from initial referral to admission through to discharge, working strategically to partner with clinical institutes and consultant accords to enhance the patient access to care triaging and directing patients care across CCL.
Addresses and resolves items impeding diagnostic or treatment progress. Participates in Data Collection and inputting e.g., Avoidable Days, Readmissions, challenges LOS where appropriate.
Will work strategically to partner with clinical institutes and consultant leaders across CCL to enhance the Patient access to care appropriately triage and direct patient care across CCL
Will ensure and support the lead case manager providing assurance that the quality delivered is compliant with regulatory requirements and Key Performance indicators
What we need from you?
Current registration through Nursing & Midwifery Council (First Level) required
Bachelor\'s Degree in Nursing required.
Evidence of working within case management
Strong organisational, time management, analytical and data management skills.
Ability to coordinate your workload and supervise members of the team, ensuring high quality, safe and effective patient care.
