Working in partnership with other statutory, voluntary, and private sector organisations to identify carers throughout Pembrokeshire
Provide information and support to carers enabling them to better manage the impact of their caring role
To lead the delivery of the Carers Support Pembrokeshire Service
Work with partners to ensure carers are supported to access other existing services.
AREAS OF RESPONSIBILITY
To provide excellent leadership for the Pembrokeshire Carers Support Service and oversee the day-to-day operations to include staff management, resource allocation and performance monitoring.
To collaborate with colleagues to identify opportunities for service development aligned with local, regional, and national policies by collating evidence of need which includes demand and capacity.
To work with colleagues to continuously improve service quality, implementing evidence-based practice and by staying up-to-date with information about best practice. This includes regular attendance at the Carers Trust Communities of Practice, their events and webinars.
To manage and supervise staff members and ensure effective caseload management by conducting 1:1 supervision sessions, appraisals, group reflective practice sessions to safeguard the well-being of staff and to assist their professional development.
To manage a case load of carers providing information, support, and assistance & outreach across Pembrokeshire to enable carers to better manage the impact of caring on their own health, financial and social situation.
When other services are more appropriate to meet the needs of carers, signpost and refer carers on to other agencies, organisations, and services, including social care for Community Care & Carers Needs Assessments.
To support carers to think innovatively about short breaks by ensuring that they are providing with information on taking a break, including opportunities to take time for themselves. This includes information on health & wellbeing opportunities in the county and the discounts and concessions available.
To support carers to research and access local, regional, and national grants to which they may be eligible and may support them to continue in their caring role e.g., Carers Support Fund and Amser microgrants.
To support carers to consider and document contingency care plans, should the carer be unable to provide carer. This will include considering wider family and social support networks and referrals to the Carers Emergency Back-Up Scheme.
To support the application process and increase the take up of the Carers Recognition Card by promoting the benefits to carers.
To identify new carers within Pembrokeshire and actively support new and existing carers to take up services provided by statutory and voluntary organisations e.g. Pembrokeshire County Council, welfare benefits advice, support groups, etc.
To work with partner agencies and others who may have contact with carers in their caring role, to increase the number of carers known to the service and to other services in Pembrokeshire. This may include activities such as attendance at events, manning stalls, providing briefings for professionals, attending drop-ins, manning points of contact in surgeries, hospitals, etc.
To support colleagues to develop communication and referral pathways with other professionals who are likely to have contact with carers e.g. CRTs, GP's, health and social care staff to ensure they are aware of the project and the services it can provide in the designated areas.
To be responsible for producing content for the Carers Newsletter, working with the Pembrokeshire Carers Support Service team and partners such as the West Wales Carers Development Group to ensure that carers are informed of the opportunities available for them.
To provide information and support to carers to enable them better to manage the impact of caring on their own health, financial and social situation.
Where other services may be more appropriate to meeting the needs of a carers make referrals to these services - as agreed with the individual carer.
To have overall responsibility for organising carers activities in the county to include but not limited to: drop-in sessions at community events, training to provide access to resources or teach new practical skills.
To work with team colleagues to identify and map out existing carer resources across Pembrokeshire and any gaps in service provision.
To work with colleagues to identify funding opportunities to enhance the service provision of the Pembrokeshire Carers Support Service.
To raise carer awareness through involvement in carer and related community events e.g., Carers Week and Carers Rights Day.
To regularly attend the Pembrokeshire Carers Provider Forum, and in the event other staff are unable to attend, the Pembrokeshire Carers Forum.

Experience

Qualifications - Essential
Excellent standard of general education or ability to demonstrate relevant experience.
NVQ/QCF Level 4 in a relevant subject e.g. Health & Social Care
ILM in Leadership and Management (or similar) or a willingness to work towards this.

Excellent communication and interpersonal skills
An empathetic and supportive manner and an ability to deal with queries from individuals who are distressed.
An ability to communicate complex information regarding carers rights over the telephone, in writing or by email

An understanding of the statutory and third sector support service provision in Pembrokeshire.
Other - Essential
Valid Driving Licence and access to a vehicle for work purposes.
