We are seeking a dedicated and compassionate Care Navigator to join our team. The Care Navigator will play a crucial role in guiding patients through their healthcare journey, ensuring they receive the appropriate services and support. Duties

Serve as the primary point of contact for patients, providing guidance and support throughout their healthcare experience. Assess patient needs and coordinate care plans tailored to individual requirements. Facilitate communication between patients, healthcare providers, and community resources to ensure seamless care transitions. Maintain accurate records of patient interactions and care plans in compliance with privacy regulations. Educate patients about available services, resources, and treatment options to empower informed decision-making. Collaborate with multidisciplinary teams to enhance patient care and address any barriers to access. Monitor patient progress and follow up as necessary to ensure adherence to care plans.

Experience

Previous experience in a healthcare setting is preferred, particularly in roles involving patient interaction or care coordination. Emis Web software is also preferred. Strong organisational skills with the ability to manage multiple tasks effectively in an office environment. Excellent communication skills, both verbal and written, with a focus on empathetic patient engagement. Proficiency in using electronic health records (EHR) systems or similar software is advantageous. A commitment to continuous professional development and staying informed about healthcare trends and resources.
