The Care Coordinator plays a vital role in ensuring that patients receive comprehensive and coordinated care. This position involves working closely with healthcare professionals, patients, and their families to facilitate effective communication and support throughout the care process. The ideal candidate will possess strong organisational skills and a keen attention to detail, ensuring that all aspects of patient care are managed efficiently.
Duties
Act as the primary point of contact for patients, addressing inquiries and providing information regarding their care plans.
Coordinate appointments, tests, and follow-up visits to ensure seamless patient experiences.
Maintain accurate and up-to-date patient records in compliance with confidentiality regulations.
Collaborate with healthcare providers to develop and implement individualised care plans tailored to each patient's needs.
Monitor patient progress and communicate any changes or concerns to the appropriate medical staff.
Assist in the preparation of reports and documentation related to patient care activities.
Facilitate communication between patients, families, and healthcare teams to promote understanding and engagement in the care process.
Supporting the carers on the field and working alongside the carers
Other responsibilities as assigned by the Registered Manager
Skills
Strong organisational skills with the ability to manage multiple tasks effectively.
Excellent communication skills, both verbal and written, enabling clear interactions with patients and healthcare professionals.
Proficiency in office management tasks, including scheduling, record keeping, and data entry.
Ability to work collaboratively within a team environment while also being self-motivated.
A compassionate approach towards patient care, demonstrating empathy and understanding of their needs.
Familiarity with healthcare systems and terminology is advantageous but not essential.
Requirements:
NVQ Level 2 or 3 in Health and Social Care or equivalent qualification
Previous experience in a supervisory or leadership role within the care sector
Strong leadership and communication skills
Excellent organizational and time management abilities
Compassionate and empathetic approach to care
Ability to work effectively under pressure and prioritize tasks
Access to a vehicle and valid driver's license essential
Willingness to undergo relevant background checks
This role is essential for enhancing patient experiences within the healthcare system while ensuring that all administrative aspects of care coordination are handled proficiently.
