Responsibilities

Deliver comprehensive assessments and interventions for patients, ensuring adherence to best practice guidelines. Collaborate with healthcare professionals to develop and implement patient care plans tailored to individual needs. Maintain accurate and up-to-date patient records, documenting all interactions and interventions. Provide education and support to patients and their families regarding health management and treatment options. Participate in clinical audits and quality improvement initiatives to enhance service delivery. Supervise and mentor junior staff members, fostering a supportive learning environment. Engage in continuous professional development to stay abreast of current practices and innovations in healthcare.

Skills

Strong clinical assessment skills with the ability to analyse complex information. Excellent communication skills, both verbal and written, with the ability to engage effectively with patients, families, and colleagues. Proven organisational skills with the ability to manage multiple priorities in a fast-paced environment. Experience in working within a multidisciplinary team, demonstrating collaboration and teamwork. Knowledge of relevant legislation and policies affecting healthcare delivery. Proficiency in using electronic health record systems for documentation and reporting purposes.
