• Conduct early assessment and intervention for patients in the Same Day Emergency Care Unit
• Facilitate safe patient discharge to their normal place of residence
• Participate with the Frailty Therapy Team across three frailty wards
• Engage in an integrated therapy approach to offer comprehensive and holistic assessments
• Deliver high standards of patient care
• Provide therapy support within the Same Day Emergency Care Unit under an integrated model
• Operate as an autonomous practitioner while working collaboratively with the Multidisciplinary Team
• Adopt a proactive approach to rehabilitation to maintain and improve patient function
• Promote timely discharges with a focus on "Home First"

Requirements:

• HCPC registration required
• Minimum of 2 years of experience
• Car driving preferred
