An exciting opportunity has arisen for a highly motivated, experienced nurse to join our Critical Care Outreach Team. This Band 6 Critical Care Outreach Practitioner post is an excellent opportunity to advance your clinical knowledge and develop clinical examination and diagnostic skills.
Our Critical Care Outreach Team is a proactive 24-hour service that provides early recognition, assessment and management of the deteriorating patient Trust-wide. We support all aspects of the acutely and critically ill patient pathway including timely admissions to an intensive care bed when required, delivery of effective follow up for patient's post-discharge from intensive care and educational support to enhance skills and knowledge of multidisciplinary ward teams caring for the at-risk and deteriorating patient. The Hospital @Night coordinator role is a fundamental part of the out of hour's team, providing expert advice, triaging and effective and timely allocation of tasks.
We are looking for enthusiastic and dedicated individuals with excellent assessment, communication and management skills who will provide outstanding high quality patient care outside of the critical care area. Improving patient safety is integral to the role whilst developing advanced clinical skills in line with a national standards and competency framework. The post holder may be required to work elsewhere within the Critical Care Units on all sites.
Main duties, tasks & skills required

The post holder will become a skilled member of the Critical Care Outreach Team for the span of duty under the supervision of a Senior Outreach Practitioner forming an integral part of the internal urgent and emergency response team (resuscitation).
The post holder will develop advanced clinical assessment and diagnostic skills to detect and provide early intervention to the acutely/critically ill and deteriorating patient. Under supervision the post holder will develop specialist clinical skills within limits of clinical competency.
The post holder will be responsible for the management of a case load of patients referred to the Critical Care Outreach Team and the appropriate deployment of the multi-disciplinary team via the iBleep (or equivalent) system when in use.
The post holder will be expected to participate in a training and development programme to acquire advanced clinical assessment and diagnostic skills to formulate and implement plans of clinical care for the acutely/ critically ill and deteriorating patient. Assist in the promotion of learning opportunities and the development of clinical skills for all grades of staff within the Trust.
The post holder will manage Level 2 and 3 patients outside of the Critical Care Units as required, in conjunction with the Critical Care multidisciplinary team. Transferring patients to appropriate areas when beds are identified - internally or externally to the Trust.
