Adults Social Worker - Ongoing Support Team West
Wiltshire Council
Pay rate - \xc2\xa335 an hour (Umbrella)
3 month initial contract

• We require an experienced Social worker to join our Ongoing support team in the West of the county.
• Ongoing Support Teams (OST) in adult care, made up of social workers, occupational therapists, and social care practitioners, with a Team Manager, Assistant Team Manager and Business Support Coordinator in each.
• We are a large, busy team, supporting adults with issues they are facing because of a disability or health condition or, sometimes, because of their social circumstances.
• The teams include expertise across a range of areas, for example, in working alongside people living with dementia (including increasing numbers of \'younger adults\'), complex neurological conditions (MND, MS, Huntington\'s, for example), Acquired Brain Injury (ABI) substance misuse, end of life support, physical disability, mental health, homelessness and learning difficulty to name a few.
• Staff are hugely adaptable and are able, due to the very different types of need they come across throughout each day, to use their skills and experience to work in a person-centred way.
• Skills include completing Care Act Assessments, complex moving and handling skills, minor and major adaptation work, assessing fluctuating mental capacity, working with people at significant risk, crisis management, and working with people as part of safeguarding enquiries.
• A lot of the work is underpinned by the Mental Capacity Act, with a real focus on prevention and strength-based working.
