4 face-to-face assessments across 1 or 2 days in Norfolk, with remaining days remote working (37.5 hours per week)

We are looking for a qualified, experienced social worker who wants to make a positive difference to the lives of people they work with. This role is flexible and how you split your time is up to you!

At Xyla Health and Social Services, we pride ourselves on delivering quality outcomes for service users and our clients. Our team of social workers and quality assurers are supported by our Senior Social Work Lead, providing the practical support you need and CPD opportunities. As part of this project, you\'ll also be supported by our Project Co-ordinators and Head of Operations.

You\'ll be completing face-to-face Care Act assessments on behalf of our client across the Norfolk area. A minimum of 4 assessments are to be undertaken per week, with flexibility on which days you choose to do these (subject to service user availability). Service users will be living in the community, care homes or other intermediate care settings within the county of Norfolk. When you\'re not visiting service users, you can work remotely.

Where the service user requires formal care, you\'ll complete the Care & Support Plan with the person and help broker the services required, either from community assets or commissioned services. Where required, you\'ll also complete 6-week reviews, Mental Capacity Assessments, Best Interests Decisions and CHC Checklists.

A strengths-based, person-centered approach is paramount to the completion of the assessments and Care & Support Plans. You will use your professional understanding and analysis of a person\'s health and care needs, to make an eligibility determination under the Care Act. Supporting informal carers and connecting the person to local community assets and/or providing formal care where appropriate to meet those eligible needs. You\'ll also need to safeguard the human rights of those individuals, reducing risk of harm and promoting ideal, independent outcomes.

Everyday you will...

  • Complete assessments to a high standard and in line with the care act
  • Work with Xyla Quality Assurer to ensure assessments and actions are progressed in a timely manner
  • Complete a minimum of 4 Assessments/reassessments per week on a face to face basis with all other work being completed remotely
  • Provide important safeguards for the human rights of those individuals, reduce risk of harm and promote ideal, independent outcomes.

To thrive in the role you must...
  • Be a Qualified Social Worker - BA Honors or MSc in Social Work
  • Be registered with Social Work England without any restrictions
  • 2 year post qualifying experience of completing assessments and reviews under the Care Act and have relevant practice skills around using the Mental Capacity Act with evidence of continuing professional development within these areas of practice
  • Have up to date training by a recognised provider in Safeguarding, Applying the Mental Capacity Act & Diversity & Inclusion and legal updates.
