North Somerset Council is currently seeking an experienced Adults Social Worker to join our team, focusing on hospital discharge. This is a temporary position starting on 2nd January 2024 and concluding on 26th March 2024.

Key Responsibilities:

Conduct assessments and create care plans for adults undergoing hospital discharge.
Collaborate with healthcare professionals, service users, and their families to ensure effective care transitions.
Work within the framework of relevant legislation, policies, and procedures.
Provide support and guidance to individuals to promote independence and well-being.
Requirements:

Qualified Social Worker with relevant professional registration.
Previous experience in adult social work, particularly in hospital discharge.
Knowledge of relevant legislation and policies.
Strong assessment and care planning skills.
Excellent communication and interpersonal skills.
